Context: A large healthcare system frequently receives patients who have been treated by multiple providers, some outside of its network. These patients bring with them a variety of clinical documents in CCDA format, such as medication lists, problem lists, and recent procedure reports. Managing and integrating this information to derive meaningful insights poses a significant challenge. The information is often siloed, making it difficult for healthcare providers to get a comprehensive view of a patient’s medical history. This fragmentation can lead to missing patients during segmentation processes, leading to lower outcomes.
Scenario: A healthcare organization aims to improve its chronic disease management programs, focusing on conditions like diabetes and heart disease, which are prevalent among its patient population.
